Forms & Trip Paperwork

All scouts and passengers must fill out the trip information and release form prior to boarding. If a scout/passenger is under the age of 18, then the forms must be signed by the parent or legal guardian. Be sure to have any scout/passenger that is traveling without their parent/legal guardian get this signed before departure.
If you would like to participate in our shark dive, please print out the shark release form and have this signed as well. Please fax all copies to us 2 weeks prior to departure. Bring originals to the boat.
Leaders may use this BUNK CHART to make sleeping arrangements for everone on board. This form also needs to be sent to the office 2 weeks prior to departure so that we may be properly prepared for your arrival! Please note any dietary needs or special occasions here.
